Baker Baker Health is run by a fellow GP and provides specialist GPs to deliver medical assessments for both Children in Care and their carers. 

We are currently operating in the following counties:

We understand how busy General Practice can be and our goal is to work efficiently alongside you to help the young people and their carers to thrive while making your job as easy as possible.

Below are the medical assessments we offer and how we may collaborate with you:

 

Initial Health Assessments (IHAs)

 

Initial Health Assessments (IHAs) are statutory medicals for children entering care, with a statutory timeframe of being completed within 28 days of coming into care. These assessments are crucial for identifying and addressing the young person’s needs. By doing so, they help the child to recover and thrive in life. They can also help reduce the likelihood of future healthcare demands on your practice.

 

How we collaborate with you on IHAs:

  • Medical Records: We may access SystmOne directly or request records via email, depending on the data-sharing agreements in your county. A prompt response with the child’s electronic medical records would be greatly appreciated in helping us to meet the statutory time requirements.
  • Reports and Action Plans: Once the IHA is complete, we will send you the full report and a letter outlining the care plan. Any actions that require your attention will be clearly marked, but we will mostly encourage carers to contact you directly. We will also inform you about the correct SNOMED codes to record the medical assessment.
  • Reducing your workload: We are fellow GPs and will do our best to reduce demands on you, for example initiating treatments ourselves where possible and just asking you to kindly follow up how they are getting on.
  • Resources for Children in Care and their Carers: we have a wealth of age appropriate blog articles addressing common issues which we refer to at medicals. You may find them useful to direct patients to during your clinical work

Adult Health Reports (AH)

 

Adult Health Reports (AH) are typically conducted by patients own GPs, but in some areas, commissioners are turning to specialist services like ours for the following reasons:

  • Reducing demand on GPs
  • Faster turnaround times, allowing faster placements of children into long term foster placements
  • Specialist insights in the report on the requirements for carers.

How We Collaborate with you on AH Medicals:

  • Medical Records: Depending on the data-sharing agreements, we may extract the carer's medical history from SystmOne directly or request an electronic copy via email with their written consent. A swift response with their electronic medical records would be greatly appreciated.
  • Health Concerns: If any potential health issues are identified during the medical, we will, where appropriate, refer the carer back to you. However, we aim to provide initial advice on self-management and lifestyle improvements whenever possible to minimize the burden on your practice.
  • QoF data: The reports will contain some useful data which may be used for QoF which will be clearly identified.
